Obtain a Copy of Your Birth Certificate

The Berwyn Public Health Department maintains official birth records, so you’ll need to contact their vital records department to obtain your birth certificate. The Berwyn Public Health Department can be contacted at (708) 788-6600.

Request Your Medical Records or Medical Transcripts

In order to protect your privacy, MacNeal Hospital requires written authorization and proof of your identity to release your medical records to another person or organization. Below is the Authorization for Release of Medical Record Information to Use and Disclose Individually Identifiable Health Information, which you can print, fill out and return to the Medical Records department or fax to (708) 783-3171

Request Access to Patient Portal

Did you stay in the hospital overnight (inpatient)?   You can obtain your medical information electronically. In order to protect your privacy,  MacNeal Hospital requires written authorization and proof of your identity to provide access to your Patient Portal medical information.  An email address is required.

HOW MUCH DOES IT COST TO OBTAIN A COPY OF MY HEALTH INFORMATION?

There is no charge for releasing copies of health information directly to health care providers. The charge to release copies of health information directly to you, or your representative, is based on the Illinois State Comptrollers guidelines, which are updated annually.

ONCE MY MEDICAL RECORDS REQUEST IS APPROVED, WHEN WILL I RECEIVE MY MEDICAL RECORDS?

Your medical records will be processed as quickly as possible. Medical Records Department is allowed 30 days to respond to your request. When requested, you can choose to pick up your medical records at MacNeal Hospital Medical Records Department or have your medical record copies mailed to the address you provide on the request for your records.
